Opening hours
Diyala Psychology Care is open weekly from Tuesday to Friday, 9:00 to 17:00. By arrangement it is possible to schedule an appointment outside regular opening hours. A surcharge of € 20 per hour applies for this.
A long-term or structural change to opening hours will be communicated via the website. Clients will be informed individually where necessary.
Availability
Diyala Psychology Care can be reached by email at info@diyalacare.nl or by phone on 06-58016913 (also via WhatsApp). Please note that telephone availability may be limited due to appointments with clients.
Registration and procedure
Diyala Psychology Care uses the following fixed procedure: prospective clients can register by completing the contact form on the website. After completing the contact form you can immediately schedule an appointment for a phone call of no more than 15 minutes, in which we determine whether Diyala Psychology Care can be of value to you. If so, we can schedule an intake session right away, and you will receive an email with general information and instructions for the first appointment.
If you cannot or do not wish to schedule an intake session right away, you have up to four weeks after the phone call to get in touch to make a follow-up appointment. If you do not get in touch within this period, the registration process ends and all information you have provided is deleted.
The intake session can take place at the location or online, as can all subsequent appointments. At the end of the intake session we draw up a treatment plan together. After completing the treatment plan I will ask you to sign it, and we can schedule a follow-up appointment. After completing treatment you receive an evaluation form on which you can indicate how you experienced the treatment.
Ending the treatment agreement
The treatment agreement can end in three ways:
- 1The treatment agreement ends automatically once treatment is completed. The therapist will inform you of this at the latest during the second-to-last session.
- 2As a client you are entitled to end the treatment agreement early and unilaterally with immediate effect by informing Diyala Psychology Care by email. This does not affect any outstanding payment obligations.
- 3Diyala Psychology Care is entitled, in accordance with Article 7:460 of the Dutch Civil Code, to terminate the treatment agreement if there is a compelling reason to do so.
Payment
By scheduling an appointment – other than the first free call after completing the intake form – you enter into a payment obligation. As a client you are and remain the debtor of Diyala Psychology Care, even if arrangements have been made between Diyala Psychology Care and your employer regarding reimbursement of costs.
After treatment you receive an invoice by email within 7 days. Diyala Psychology Care applies a payment term of 14 days. If you have not paid the invoice after 14 days, you receive a free reminder with another 14 days to pay. If the invoice is not paid after the second payment term has expired, Diyala Psychology Care is entitled to take collection measures and to suspend further treatment until all outstanding payment obligations have been met.
No-show policy and arriving late
If you are unexpectedly unable to attend a session, you are requested to cancel your appointment at least 48 hours in advance. Diyala Psychology Care reserves the right to charge the cost of the scheduled session if you have not cancelled your appointment or have not cancelled in time.
If you arrive late for an appointment, the lost time is deducted from the length of your session, without a reduction in the fee. You always have the option to convert an in-person appointment into a digital session; preferably let this be known at least two hours in advance so that any technical problems can be resolved in time.
Medical file and personal data
Diyala Psychology Care is legally obliged to keep a medical file. This file is stored on an external server, so the data is not stored on the practice's computer. The statutory retention period for this file is in principle 20 years. For a detailed explanation of how we handle your personal data, please consult our privacy statement.
As a client you have the right to inspect or supplement your medical file. You are also entitled to a copy of your file against an administrative fee, and you may request destruction of your file. Diyala Psychology Care will comply with the latter request, unless a situation arises as referred to in Article 7:445 paragraph 2 of the Dutch Civil Code.
Diyala Psychology Care is a member of the Netherlands Institute of Psychologists (NIP) and is therefore bound by the NIP professional code. Furthermore, the care falls within the scope of, among others, the Medical Treatment Agreement Act (WGBO) and the Healthcare Quality, Complaints and Disputes Act (Wkkgz), and work is carried out in accordance with the reporting code for domestic violence and child abuse.
